Case 138:   Metastasis - Ovarian Cancer

Clinical Presentation:

80 year old woman with pancreatic adenocarcinoma.

CT: Capsular metastases causing "pseudocirrhosis"

Imaging Findings:

CT shows capsular implants (undulating capsular contour), ascites, and peritoneal implants (arrowheads) creating the appearance of a cirrhotic contour ("pseudocirrhosis") and portal hypertension: however note that liver capsule demonstrates scalloping and concave deformities, rather than a convex nodularity like it is usually seen in cirrhosis or pseudocirrhosis secondary to parenchymal metastatic disease or other fibrotic processes. Ascites is malignant and not a complication of portal hypertension.
