Case 164:   Schistosomiasis (Bilharzia)

Clinical Presentation:

53 year old Egyptian man with cirrhosis.

CT: Periportal fibrosis, dominant in the right lobe, cirrhosis.

Imaging Findings:

CT shows cirrhosis due to hepatic schistosomiasis. Note prominent hypodense and hypovascular periportal tracts secondary to severe periportal fibrosis. Equilibrium phase would typically show delayed periportal contrast enhanced (not available). Intrahepatically small caliber portal venous branches due to encasement by periportal fibrosis. Shrunken right hepatic lobe. Status post splenectomy (splenule).
