Case 167:   Steatosis / Steatohepatitis

Clinical Presentation:

56 year old woman with necrotizing pancreatitis.

CT: Geographic mainly perivascular and subcapsular distribution pattern of fat

Imaging Findings:

CT shows irregular subcapsular distribution of fat mainly in the left lobe, but also along the medial contour of the right lobe. Note non-displacement of vessels within the focal fatty change (typical "vessel penetration sign"), distinguishing this entity from most types of neoplasms.

Note small thrombus in the main portal vein, in this patient a complication of acute pancreatitis.
