Case 178:   Perfusion Abnormality (THAD/THID)

Diagnostic Category:

Clinical Presentation:

36 year old woman with right upper quadrant pain.

CT: Hypervascular THAD due to acute cholecystitis

Imaging Findings:

CT demonstrates impacted gallstones and gallbladder wall edema as well as strong hypervascularity of adjacent liver parenchyma in segments 4B and 5 (THAD). This THAD may be due to reactive hyperemia, or to hepatic inflammation from a draining aberrant vein of the gallbladder into segment 5.
