Case 180:   Perfusion Abnormality (THAD/THID)

Clinical Presentation:

55 year old man with cirrhosis and HCC.

CT: Bile duct injury.

Imaging Findings:

Cirrhosis and HCC, status post radiofrequency ablation. CT demonstrates a peripheral focal zone of transient hypervascularity (THAD) predominantly in the arterial phase in the right lobe, immediately lateral to the tubular ablation track surrounding a dilated subsegmental bile duct. The bile duct sustained thermal damage during track ablation following the procedure. Note the large locally recurrent slightly hypervascular HCC with washout at the medial portion of the large ablation zone located posteriorly in the right lobe.
