Case 19: Chlonorchiasis

Clinical Presentation:

71 year old Korean man with fever.

CT: Small periportal cysts

Imaging Findings:

CT shows mild bile duct dilatation and numerous small hypodense lesions of similar size, in typical peribiliary location, representing infection with liver flukes. Worm is being ingested and ascends to the liver via the bile ducts. Small lesions represent encapsulated parasites (metacercariae of Chlonorchis sinensis).

Note that cysts are better appreciated on MRCP of this case.
