Case 210:   Hepatocellular Carcinoma (HCC)

Clinical Presentation:

62 year old woman, with chronic hepatitis C infection, found to have a hepatic mass on screening ultrasound (not shown)

MR: Central scar and pseudocapsule

Imaging Findings:

MR demonstrates HCC with central scar and pseudocapsule. On T2 W image, the mass appears mildly hyperintense to the hepatic parenchyma.

On the arterial phase, the mass demonstrates intense homogeneous enhancement.

On the portal venous phase and equilibrium, the mass demonstrates wash out with progressive central enhancement, compatible with a central scar.  There is also enhancing rim at the margin of the tumor at equilibrium " pseudocapsule" , consistent with peritumoral fibrosis which is characteristic of HCC.
