Case 214:   Echinococcosis, cystic (hydatid)

Clinical Presentation:

47 year old woman with right upper quadrant pain and anorexia.

CT: WHO type CE3: "snake sign"

Imaging Findings:

Portal venous phase CT shows large hydatid cyst in the dome with poorly seen collapsed membranes ("snake sign"). The membranes represent the endocyst of the parasite and are not well seen on CT, since they don't enhance. Compare to cystadenoma, where the septations typically enhance. Similar smaller hydatid cyst in the left lobe.
