Case 235:   Cholangiocarcinoma

Clinical Presentation:

51 year old man with chronic hepatitis C infection. Screening CT revealed a lesion in the right hepatic lobe.

CT: In cirrhosis, with typical early ring enhancement

Imaging Findings:

CT shows a hypovascular biopsy proven well differentiated cholangiocarcinoma with subtle peripheral ring enhancement, best seen on the portal venous phase. Note that the perifocal edema seen on MR, is not identified on CT. This cholangiocarcinoma does not show the typical desmoplastic stroma reaction on the equilibrium image obtained 5 minutes after iodine contrast administration.
