Case 256: Yttrium 90 Radioembolization

Clinical Presentation:

75 year old man with multifocal HCC status post two cycles of Y90 radioembolization.

MR: HCC - early appearance post Y90 embolization

Imaging Findings:

Corresponding to the hypervascular lesions seen on the pre-treatment CT, MRI shows innumerable slightly hypointense lesions on T1w within the liver, which demonstrate ring enhancement reflecting treated HCC. There are scattered areas of geographic arterial enhancement
surrounding these lesions within the right hepatic lobe without evidence of washout on portal venous or delayed images, but with prolonged fibrotic enhancement, representing treatment related perfusion anomalies and fibrosis.
