Case 29:   Cholangiocarcinoma

Clinical Presentation:

60 year old woman presents with abdominal pain and jaundice.

CT: With extracapsular extension and metastatic omental caking

Imaging Findings:

Multiphase CT reveals a large infiltrating central cholangiocarcinoma involving the portal structures, with exophytic extension involving the hepatic flexure and gastric outlet (arrowheads on coronal images). Tumor demonstrates delayed peripheral enhancement and central necrosis, typical of cholangiocarcinoma. Note associated omental metastatic caking and metastatic gastrohepatic nodes (arrows). Biopsy of the liver lesion confirmed cholangiocarcinoma.
