Case 294: Yttrium 90 Radioembolization

Diagnostic Category:

Clinical Presentation:

61 year old man with hepatocellular carcinoma in hepatic segment VI, post Y90 radioembolization.

CT: Hepatic infarct after Y90 radioembolization with fibrotic enhancement

Imaging Findings:

CT three weeks after Y90 treatment of small peripheral HCC with associated portal venous thrombus in the cirrhotic liver (see US prior to treatment) shows a hepatic infarct with strings of portal venous enhancement representing non irradiated parenchyma around portal triads. Progressive enhancement fills the fibrotic parenchymal portions on equilibrium phase.

Metallic markers indicating the location of the mass prior to treatment were placed in a different institution before treatment plan was changed to do Y90 radioembolization.
