Case 66:   Hemangioendothelioma, epithelioid (adult)

Clinical Presentation:

37 year old man with indeterminate liver mass who presents with abdominal pain.

CT: Atypical, large, no capsular retraction

Imaging Findings:

Diagnosis is atypical epithelioid hemangioendothelioma (EHE). Mass is relatively large and solitary;  EHE classically presents with multifocal smaller masses. Capsular retraction in this case is not or only barely present, which is atypical, however delayed peripheral enhancement is noticeable. Note small hypervascular transient hepatic attenuation difference (THAD) anterior to the lesion.
