Case 98:   Hepatocellular Carcinoma (HCC)

Clinical Presentation:

65 year old man with hepatitis C and cirrhosis presents with jaundice.

CT: With microscopic fat content

Imaging Findings:

Mutiphase CT reveals a cirrhotic liver and an enhancing HCC that is hypodense in all phases. Note that when a region of interest is drawn over the mass (see lower images), the minimum non-contrast density is negative 31 Hounsfield units, suggesting lipid content. This fat content is not evident looking at the average density of 25 Hounsfield units. Ultrasound guided biopsy revealed well differentiated HCC with fatty components. Other lesions with similar appearance were also present in this patient (not shown).
